Appoint a Wyoming registered agent
Required of a DAO exactly as of any other Wyoming LLC, and the agent must keep a physical Wyoming address. You do not have to live in Wyoming yourself.

Choose a name carrying both indicators
The name must contain “DAO” or “LAO”, and separately must carry a limited-liability-company indicator — “LLC”, “L.L.C.”, “Limited Liability Company” and the other accepted abbreviations all qualify.

Deploy the smart contract that operates the organization
This is the step this application performs. The address it returns is the publicly available identifier the articles have to name, and it stays constant as you register further contracts against it.

Decide how the organization is managed
The articles must state how the DAO is managed by its members, including how much of that management is conducted algorithmically. A DAO differs from a standard LLC precisely in being able to answer “partly by code”.

File the Articles of Organization
Online, start a Limited Liability Company and pick “Decentralized Autonomous Organization” as the Additional Designation in step 1, then follow the DAO-specific instructions in step 6. On paper, mail the DAO LLC Articles of Organization form with a check or money order.

Include the Notice of Restrictions on Duties and Transfers
It goes in the articles, or in the operating agreement with a statement in the articles saying that is where it lives. One or the other is required — not neither.

File the annual report each year after
The report keeps the entity in good standing. Its fee is assessed on assets located and employed in Wyoming.

$100 initial filing fee. Annual report thereafter, minimum $60 — assessed as $60 or two-tenths of one mill ($.0002) on the dollar of assets located and employed in Wyoming, whichever is greater.
